    Cualquier ictus, puede dar lugar a un síndrome confusional transitorio que no dura más de 4 días. Cuando los síntomas persisten en el tiempo es porque el infarto tiene una localización específica, como los infartos de la arteria cerebral posterior izquierda o bilateral que afectan al giro occipitotemporal medial o cuando afectan al tálamo, y los de la arteria cerebral media derecha que afecten al área parietal posterior y a la región prefrontal. La ruptura de un aneurisma de la arteria comunicante anterior puede dar lugar a delirium por afectación del sistema límbico.
    Otras alteraciones cerebrovasculares que pueden dar delirium son la hemorragia subaracnoidea, las vasculitis del sistema nervioso central como el lupus eritematoso sistémico y el síndrome de Behcet  y la encefalopatía hipertensiva. Las lesiones isquémicas subcorticales son factores predisponentes para el desarrollo de delirio.
